    Dr. Laura White is a board-certified pediatrician with over 20 years of experience, and as a mother of nine children, she deeply understands parental concerns and the unique needs of adoptive families

  • Erin E. Chalmers, PA-C

    Erin joined Future Hope Pediatrics in September 2015, bringing her dedication to helping children grow to their full potential and her strong interest in homeopathic medicine to the practice. She is a valued member of the team, proud to call Kentucky her home, and is excited to contribute to a practice that embodies her beliefs.

  • Taylor Burge, PA

    In 2024, Taylor became a board-certified Physician Assistant and joined Future Hope Pediatrics. Taylor believes that early childhood and adolescent years are the most wonderful and important years to shape young hearts and minds. She truly enjoys seeing all pediatric patients and working alongside other great providers! 

  • Avian Schafer, MSN | FNP | RN | BSN

    Avian joined the Future Hope family in July 2022. She was born and raised in Louisville, Kentucky where her and her family still resides. Avian and her husband Dylan have two amazing children, Wyatt and Emmerson; and one on the way. Avian has been in the medical field since 2016 and has been in ER, oncology, integrative, and functional medicine pediatrics. Her passion is working with children and helping supporting parents navigate their child’s health. She loves the Lord and enjoys backpacking, gardening, being a mom.

    Taylor joined Future Hope Pediatrics in 2025 after graduating from the University of Colorado’s Physician Assistant/Child Health Associate program. She is passionate about working alongside families, empowering both parents and children to feel confident in their medical and parenting decisions while integrating more natural solutions. She is looking forward to fostering long-term relationships with families, cherishing the opportunity to watch children grow and to be a trusted presence in their lives. In her free time, she enjoys live music, cooking, and spending time with her husband and sweet pup.

  • Yes, we accept Medicaid from the "Passport Health Plan."

  • No - the pediatricians at the hospital will conduct the initial evaluation of your baby. After discharge, we will see your child within the first few days once you are back home.

  • 1. Fill Out New Patient Paperwork

    The first step to becoming a patient at our practice is to complete our New Patient Paperwork. You have two convenient options for this:

    * Online Form:
    You can fill out the necessary information electronically using our secure online form. This method allows for a quick and efficient submission, saving you time during your visit.

    * Download and Print:
    Alternatively, you can download the New Patient Paperwork packet from our website. Print the forms, complete them at your own pace, and ensure that all required fields are filled in.

    2. Scheduling Your First Appointment - Leave It to Us!
    
Upon receipt and processing of your completed paperwork, one of our staff members will promptly reach out to you to schedule your first appointment. Our team is committed to ensuring a seamless and efficient registration process for you and your child!

    Important Note for Newborns:
    If your child is under 1 month of age, please contact our office to schedule your initial appointment. This will help prevent any delays in providing the necessary care for your newborn.

  • Yes, we welcome uninsured patients. The cost of paying out of pocket varies based on the type of visit:

    "Sick Visit" or "Office Visit": $75 - $200 (depending on the problem and time spent on diagnosis and treatment).

    "Well Check" or "Preventative Care" Checkup: $100 - $250.

    These are base prices and do not include additional labs, tests, or immunizations done during the visit.

  • Yes, if you pay in full at the time of your appointment, you'll receive a 20% discount on the total bill. Otherwise, the full price applies if you wait for the bill or pay only a portion during the appointment

  • Dr. White has been in private practice for over 20 years.

  • Our parenting philosophy is best described as "Attachment Parenting." We believe in tailoring the parenting style to meet each child's unique needs as we strive to train both hearts and minds.

  • We provide care from birth through 18 years old.

  • A Physician Assistant (PA) is a nationally certified and state-licensed medical professional who practices medicine on healthcare teams with physicians and other providers.
